Central mucoepidermoid carcinoma (CMEC), a rare intraosseous variant of mucoepidermoid carcinoma, was diagnosed in a 23-yearold female patient presenting with a lesion in the right posterior maxilla. Initial panoramic radiography and cone-beam computed tomography revealed a multilocular radiolucency with corticated margins, suggestive of an odontogenic cyst. Histopathological evaluation following incisional biopsy confirmed cytokeratin 7+, p63 (focal+), and mucicarmine+ staining, diagnostic of low-grade CMEC. Subsequent contrast-enhanced computed tomography and magnetic resonance imaging demonstrated cortical perforation and soft tissue extension, which were critical for surgical planning. This case highlights the necessity of early histopathological assessment of jaw lesions, even when imaging initially suggests benignity, to avoid diagnostic delays.
